Special Accommodations Form

Students who request special accommodations should meet with the Dean of Student Affairs to complete a special accommodation request form and submit supporting documentation. All supporting documentation must be within the past five years. Responses to requests may take 2 to 4 weeks once all sufficient documentation is submitted. Accommodation approvals are not retroactive, and begin the date of approval.
